Job Summary

The Nurse Practitioner Fellow functions in a learning role receiving advanced training through practical hands-on and didactic educational experiences designed to increase confidence and clinical skills. The program includes a structured family practice continuity clinic experience along with additional specialty rotations. The Fellow will serve as a primary care provider within various Lexington Medical Center Physician Practices, Community Medical Centers and partnered facilities. Under the supervision of a Physician, the Fellow will (1) assess the physical and psychosocial status of patients by means of interview, health history, physical examination and diagnostic tests and (2) interpret data, develop and implement a plan of treatment, follow through on a continuum of patient care across the health-illness continuum and lifespan within standard protocols set forth and approved by both the nurse practitioner and practice physicians.

Minimum Education: Masters or higher degree in nursing from an accredited College or University (obtained within the last 18 months) or a current Nurse Practitioner transitioning to Family or Internal Medicine.
Minimum Years of Experience: 2 Years of full-time or its equivalent nursing experience providing direct patient care, obtained within the previous 5 years
Substitutable Education & Experience (Optional): None.
Required Certifications/Licensure: Advanced Practice Registered Nurse (APRN) licensure in South Carolina;  Certification from a South Carolina State Board for Nurse Practitioner Program;  Licensed as a Register Nurse by the South Carolina Board of Nursing; Certification from a South Carolina Board Approved credentialing organization. Preferred certification by American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP); Current Prescriptive Authority per SC State Board of Nursing
Required Training: None.

Essential Functions

  • Maintains current signed written practice agreements, reviewed and revise as needed.
  • Has contact by phone, correspondence, or personal meetings with persons in other departments or outside the practice; tact is required in these contacts and the employee assumes the responsibility for harmonious relationships.
  • Documents all situations, both internally and externally, that may arise out of the ordinary or are necessary to maintain accurate patient charts/records.
  • Helps maintain the work area in a neat and orderly fashion. Responsible for obtaining and keeping supplies necessary to perform the job duties.
  • Works directly with the Coordinator of PN Development in establishing goals and objectives and follows them through to completion.
  • The Fellow will be committed to attend and complete a scheduled sequence of educational requirements. 
  • Under supervision, Fellow provides diagnosis and treatment of common acute illnesses; prevention and maintenance health care to patients with stable chronic illnesses; and performs interim physical examinations for healthy patients within prescribed guidelines.          
    • Elicits appropriate present and past medical history and review of systems; Performs appropriate physical examination and orders appropriate diagnostic studies.   
    • Discriminates between normal and abnormal findings in history and physical; Establishes appropriate differential diagnosis. 
    • Assesses stability of chronic illness and compliance with present therapy; monitors    for complications of disease or therapy; Establishes appropriate treatment plan. Assesses need for hospitalization or physician intervention.
    • Determines timing for follow-up and orders appropriate referrals; Counsels regarding health maintenance and disease prevention, and provides anticipatory guidance as appropriate.
  • Provides health education to patients about ways to improve, promote, and maintain their health status, including but not limited to providing educational information on disease/disease processes, self-care practices, and positive lifestyle choices (i.e. diabetes, safe sex, weight loss, smoking cessation, etc.).          
    • Assesses learning capabilities and readiness of population or individuals, and tailors education to meet age, developmental, and educational needs.  
    • Prioritizes learning needs and documents them accordingly.
    • Ensures the time frame and subject matter is appropriate for target audience/individual.
    • Utilizes appropriate teaching materials (handouts, audiovisuals, demonstration, etc.), documents use, and patient outcome.
    • Indicates, designs, and completes educational programs for patients, families, and targeted audiences.
  • Maintains accurate records, medication lists, and documentation of care and follow-up for administrative purposes and reimbursement of services.
  • Adheres to agreed-upon format/protocol for documentation or records.
  • Maintains appropriate level of documentation to promote/support level of complexity when compared against/to service fee charges.
  • Reviews and updates problem list at least on annual visit/episodic visit by patient and initiates problem and medication lists on new patients seen.
  • Documents telephone calls with patients and specialists.
  • Documents visit in concise, clear, logical, and legible manner.
  • Maintains licensure/certification.
  • Identifies the knowledge and skills needed through self-assessment, peer review, and supervisory review.

Duties & Responsibilities

  • Participates in a variety of educational activities in order to maintain and increase competency.
  • Uses continuing education opportunities as a basis for expanding medical knowledge and improving clinical skills, and incorporates these into the daily practice of medicine.
  • Participates in developing, implementing, and interpreting quality assurance/risk management programs.
  • Supervises and intervenes with clinical issues and supports/directs triage by clinical staff when necessary.  Implements clinical education and training or clinical staff when necessary.    
  • Provides constructive feedback as it relates to performance, patient flow, and patient care issues.
  • Provides in-services and clinical education for staff members on an “as needed” basis.
  • Is available for feedback on call-back and triage issues as needed.
  • Identifies barriers to patient care flow or delivery, and assists at formulating corrective action.
  • Serves as a role model for patient interaction and conflict resolution for clinical staff.
  • Works collaboratively with health team members, both in and out of office setting, to ensure continuity of care and complete delivery of prescribed care.
  • Demonstrates regard for the dignity and respect for all patients, their families, guests, and representatives of other organizations as well as fellow employees, volunteers, and medical staff in support of the practice’s mission to provide consistent, quality health care services in a professional, caring, and responsive environment.
    • Maintains confidentiality of patient and of patient/practice information with no infractions as noted by supervision; Consistently display a caring and responsive attitude, represents the practice in a positive manner, and conducts all activities respecting patient/customer rights and expectations.
    • Regularly maintains a neat appearance and adheres to department/corporation dress, including the wearing of appropriate identification; Interpersonal relations with other health care workers are regularly fostered in a courteous and friendly manner as evidenced by supervisory observation and peer input.
    • Resolves conflicts with staff members by following established communication norms with limited involvement by supervisor to initiate resolution; Consistently receives and gives suggestions and constructive criticism in a professional manner.
  • Demonstrates responsibility for individual performance and efficient utilization of products, supplies, equipment, and time to ensure the timely completion of duties and to promote financial viability through provision of services at a reasonable cost.    
  • Maintains consistent level of productivity as influenced by impinging factors of access, practice load, and complexity of care.
  • Performs other support functions to enhance the general office flow, such as clerical, laboratory, and housekeeping duties.
  • Provides proper notification for all absences or tardiness, scheduled shift and scheduled time off in accordance with company policy.
  • Consistently uses products, supplies, and equipment in an efficient manner, keeping waste within practice limits as observed by peers and administration.
  • Follows all established safety precautions, policies, and procedures in the performance of all duties in order to ensure a safe environment. Practices proper infection control and universal precautions techniques.
  • Demonstrates a complete knowledge of body mechanics by consistent use in the work setting as evidenced by no injuries sustained as a result of improper body mechanics in the evaluation period.
  • Performs all other duties as assigned by authorized personnel or as required in an emergency, e.g., fire or disaster.
