Job Summary

The primary purpose of your position is to assure all residents are admitted in a timely and efficient manner, in accordance with established policies and procedures of Lexington Medical Center Extended Care, and as directed by your supervisor. The Admissions Nurse Liaison will report directly to the Director of Admissions and dotted line to the Director of Nursing.

Minimum Qualifications

  • B.S. degree in Human Services or related field.  
  • 2+ years in admissions with background in Nursing and Marketing.
  • Must be, as a minimum, a graduate of an approved RN program.
  • Required Training: Completes yearly safety training and other mandatory classes. All licensed nurses shall possess a valid Healthcare Provider cardio-pulmonary resuscitation (CPR) certificate within six (6) months of their first day on the job in the facility.  CPR classes are provided by LMC Extended Care Staff Development.

Essential Functions

  • Assume the clinical authority, responsibility and accountability of performing the assigned duties of the Admissions Nurse Liaison.
  • Keep abreast of current Medicare/Medicaid regulations governing admissions/discharge requirements of health care facilities.
  • Develop and maintain a good working relationship with inter-departmental personnel, departments within the facility and throughout the Lexington Healthcare District.
  • Maintain an accurate record of available beds for admitting proposes. 
  • Will be familiar with medical conditions and services needed for assessing patients for new admissions.
  • Will review referrals for possible admission to the facility. This will include reviewing all available medical information, interviews with medical personnel/family members/potential residents.  Making home/hospital visits as appropriate.
  • As part of the admission process:  working with admissions staff for assigning of a room number; collect, assemble and check admission papers; obtain the resident’s/guardian’s signature on all required permits, releases, authorizations, etc. Will verify Medicare, Medicaid and/or insurance coverage prior to admission of all residents.  Explain to resident and/or guardian room rates, billing procedures, visiting hours, restrictions, as appropriate. Assemble medical charts and review admission paperwork for accuracy if needed.

Duties & Responsibilities

  • Will ensure that residents are admitted, transferred and discharged in accordance with established policies and procedures.  
  • Will conduct tours of the facility for families of potential admissions and interested members of the community as needed.  Be able to provide all necessary information related to areas of the facility, i.e. care provided, staffing, cost, special services unique to certain areas.
  • Will be available for community education in areas related to long-term care, Alzheimer’s disease and related subjects.  Keep a breast of current information related Alzheimer’s disease and treatment and information related to long-term care services.
  • Will make visits to potential community referral resources for Carroll Campbell Place.
  • Other related duties and responsibilities that may become necessary to assure that the needs of the facility are met.
